Foam is a bad idea. Expensive and it doesn't work. Here's a great podcast episode that might save you money and which gives concrete instruction.

I believe the gentleman in the podcast puts the fiberglass panels into wooden frames for mounting. Actually, I think he has them made, but you could do the framing yourself I'm sure.

Yes. You can make these yourself. It isn't hard to do. I have some. You can also buy premade panels. I recommend GIK: http://www.gikacoustics.com/ I also have some of these products.
Also, consider diffusion in addition to absorption!

I saw something on reddit a while back where a guy used painting canvasses with foam behind them to make some nifty, customized panels. DO NOT DO THIS. CANVAS WITH PAINT DOES NOT ABSORB SOUND. It looks neat though.
