Make your own ringtones
I'm sure some folks already know how to do this, but for those who think it's scary, it's really not. These are the steps that work on my Verizon LG phone, and I understand that this works on most Verizon phones, and that the process is similar for other providers:
Chop your song however you like (20 seconds is probably safe for most phones, it may need to be shorter)
Export it as an mp3 at 64k, 8bit mono
Send an email to [your number]@vzwpix.com with the mp3 as an attachment
In seconds, you'll recieve a text message. You should be able to save the mp3 as a ringtone.
Now when anyone other than bandmates/gf calls me, my phone plays this , the chorus of my 'It Never Happened.'
The quality is exactly at the level of comercial ringtones, you can show the world how crappy your music is one phone call at a time, and it's FREE. I feel like a tard now for paying for ringtones on my last few phones.
Anyway, woohoo.

Most phones allow you to upload a MIDI ringtone via cable or bluetooth. Some newer ones allow you to do the same with wavs or mp3s. It all depends on the phone. Also some carriers like Cingular are pretty notorious for disabling those functions from some of their phones (but not others), and some phones (like the Danger Hiptop aka Sidekick) are completely locked-down to the point that you can ONLY get ringtones through carrier-approved channels.
Ringtones are big business (for some reason) and the carriers don't want to do anything to jeopardize that, especially on the cheaper or trendier phones. Business phones tend not to be restricted in this way, regardless of carrier.
Also if you have data service you can usually put a .mid file on your website and then download it via the crappy web browser in your phone. Again, depends on model and carrier and service plan and blah blah blah.
Another way is if you can find an email to MMS gateway you can attach the ringtone to an MMS message, though then the carrier charges you to receive the message (usually based on the attachment size).

I tried Sober's idea but the message my phone got was, "You got some picture message, use your computer to see it" so I guess my phone isn't compatible.

Has anyone done this on a Treo 650? I don't see an option to save as a ringtone.

Despite lowering the gain by 50% the MP3 still distorts an aweful lot. Plays nicely in 16 bit, but on the phone sounds like crap. Any tips on getting a cleaner sound?
